Policies: I (we authorize Fit for Me to electronically debit or charge my account each month on the indicated date (10th or 25th of each month). The electronic debit or charge will continue until Fit for Me receives written notice of termination 30 days prior to new billing month. Oral and e-mail cancellation is not accepted. Members authorize Fit for Me to debit or charge account for all sums. Oral and e-mail cancellation is not accepted.

Waiver and Release: I hereby waive and release any and all rights and claims for personal injury or loss of property while training at Fit for Me. I understand the scope and risks contemplated by this waiver and release. I assume responsibility for myself and incur any and all risks and loss, damage, or injury including death; including but not limited to the risk of harms caused in whole or part by the unintentional conduct of Fit for Me.

Guest Policy: To assure the safety and comfort of our members and employees; members may not provide or allow any other persons to have their PERSONAL KEY CODE. If your code is given to an unauthorized person, your membership will be terminated immediately. No refunds will be provided. Guest may come with member and must sign appropriate guest waiver upon arrival. Guest may have one visits per year, unless prior approval. Please notify Carrie @ 521-9388 prior to bringing in any guest.
